My wife is getting some spam, and based on the log it looks like it is because the address(es) are in the whitelist. As the whitelist is quite large, should I just delete the entire thing and start again?

Probably not. First you should determine why the addresses are in the whitelist. Are they ones you added, or ones that SpamSieve added (i.e. due to “Trained: Good (Auto)”)? If an address is incorrectly whitelisted, when you train the message as spam SpamSieve will disable the whitelist rule. So this shouldn’t be a problem for long.
